Submissions

If you submit information to the Service (including product or company details), you represent that you have the rights to share it and that it is accurate to the best of your knowledge. We may review, moderate, and display submissions as part of the marketplace.

If you submit personal information through assessments, forms, or other journeys on HSMA, you are responsible for ensuring that the information is accurate and that you are authorized to share it.

Partner Providers and Referrals

HSMA may help you discover, compare, and connect with third-party partner providers that offer weight loss services or treatment plans. Any purchase, treatment, prescription, eligibility decision, contract, or ongoing relationship is between you and the relevant provider, not HSMA.

Where you ask us to connect you with a provider, request a treatment journey, or otherwise authorize the handoff, you agree that we may share relevant information with that provider so they can contact you, assess your request, and help you continue with the service or treatment plan you selected.

Analytics and Event Tracking

We may use analytics tools, cookies, and event tracking technologies to understand how the Service is used, improve performance and usability, measure engagement and conversions, and better understand which journeys lead to provider inquiries or purchases.

Intellectual Property

The Service, including its design and content, is owned by IntentForms or its licensors and is protected by applicable intellectual property laws. You may not copy, modify, distribute, or create derivative works from the Service except as permitted by law or with our written permission.
